For paper application: You will need to complete the application and bring it to one of their offices to submit and obtain your card. If distance is a problem in getting to an office we can arrange to take students during the start of the program. However, it is preferred to have this completed before the start of the program, if possible.

Also, when you make contact with your vocational rehabilitation counselor, make sure to mention that you have obtained a UTA reduced fare card and that you need a sticker to ride for free. They can provide you with a sticker to go on your reduced fare card so you will not have to pay a reduced fare but will have free fare on all the buses for the duration of our program. Make sure to speak with them about this so they can write this needed service in your IPE plan with them.

I called UTA and was able to confirm that they will accept a signature from your vocational rehabilitation counselor instead of your eye doctor, if this is easier for you. OR, they also said they will accept a Medicare red/white/blue card for the student applying and you won’t need to complete this application. Just show up at the office with the card and the student and they will have some simpler paper work for you to fill out.

UTA’s Paratransit Service ADA program is a service of the Utah Transit Authority, for people with physical, cognitive or visual disabilities who are functionally unable to independently use the UTA fixed route bus service.
Appointments are required for a mobility evaluation. Call the Paratransit Mobility Center at 287-2263 or our toll free number is 1-877-882-7272 if you live outside the Salt Lake area. You may call Monday through Friday from 7:30 am to 4:00 pm. Upon request, complimentary transportation is provided.
